Cryptococcal antigen assays detect circulating glucuronoxylomannan capsule antigen with high sensitivity in cryptococcal meningitis and disseminated disease.

Specimen Requirements
- Specimen: serum and/or CSF (follow order)
- Tubes: microbiology typically receives an early sterile tube (follow institutional tube order)
- Handling: transport immediately to the laboratory; do not refrigerate if bacterial culture is primary (follow SOP)
- Volume: provide adequate volume for Gram stain, culture, and ancillary tests ordered
Analytical Method
Lateral-flow assay (LFA) or latex agglutination detects capsule antigen. Titers may be reported for some methods. Proneose/hook effects are addressed by dilution protocols when indicated.

Expected: antigen not detected. Semi-quantitative titers, when reported, are method-specific; use the performing laboratory’s scale.

Critical and significant-risk microbiology findings (e.g., positive blood culture, CSF pathogen) follow institutional urgent-notification pathways. Confirm the local call list. Textbook examples are not universal thresholds.
