Overview
ANC = WBC × (neutrophil fraction). When bands are reported separately, most laboratories include bands in ANC (segs + bands).
Clinical Importance
Grade neutropenia and support infection-risk assessment
Monitor chemotherapy nadirs and recovery
Specimen Requirements
- Specimen: whole blood
- Tube: lavender-top K₂EDTA (preferred) or K₃EDTA
- Anticoagulant: EDTA
- Handling: invert gently 8–10 times immediately after draw; do not shake
- Storage: room temperature preferred for prompt analysis; follow analyzer/manufacturer limits
- Stability: analyze as soon as practical (many CBC parameters are stable for several hours at room temperature; verify local procedure). MPV increases with EDTA storage time. Do not freeze whole blood for CBC.
Analytical Method
Calculated from total WBC and the neutrophil (± band) percentage from automated or manual differential. Band inclusion must match institutional reporting policy.
Instruments Used
Automated analyzers
- Sysmex XN / XN-L series hematology analyzers
- Beckman Coulter DxH series hematology analyzers
- Abbott Alinity hq / CELL-DYN hematology analyzers
- Siemens Healthineers ADVIA hematology analyzers
- Mindray BC-series hematology analyzers
- HORIBA Yumizen / ABX hematology analyzers
Manual methods
- Derived from manual differential × WBC when automated absolute counts are unavailable
Reference Values
Typical adult SI examples below follow widely published international hematology practice (e.g., Dacie and Lewis; Henry's). Always interpret against the method- and demographics-specific interval printed on the patient report (CLSI EP28-A3c). Neonatal, pediatric, pregnancy, and altitude intervals differ.
Typical published adult ANC approximates 1.5–7.5 × 10⁹/L; exact limits and band handling are laboratory-specific.
Clinical neutropenia grades (e.g., <0.5 × 10⁹/L as severe) are grading constructs; confirm institutional critical limits separately.
Interpretation
Critical and significant-risk limits are institution-defined (CLSI GP47). Confirm the local call list. Textbook examples are not universal thresholds.
Critical values
- Severe neutropenia commonly appears on critical call lists; verify local ANC threshold (CLSI GP47)
Common Causes of Increased Results
- Neutrophilia; acute stress, infection, inflammation, marrow stimulation patterns
Common Causes of Decreased Results
- Neutropenia; evaluate severity, duration, and smear for dysplasia or left shift
Clinical Significance
- Chemotherapy safety monitoring
- Fever and neutropenia pathways
- Drug-induced neutropenia surveillance
Factors Affecting Results
Pre-analytical
- EDTA artifacts, delayed processing, and mislabeled specimens
- Steroid therapy or physiologic stress can acutely shift neutrophil counts
Analytical
- Misclassification between lymphocytes and monocytes on some automated differentials
- NRBCs inflating WBC if not corrected before absolute calculations
Post-analytical
- Interpreting percentages instead of absolute counts
- Applying adult intervals to pediatric patients
Advantages and Limitations
Advantages
- Clinically actionable absolute value
- Easily tracked serially on CBC differentials
Limitations
- Band definition variability between laboratories
- Does not distinguish marrow production failure from consumption without context
Clinical Pearls
Always compute and report ANC; a 'normal' WBC can conceal life-threatening neutropenia.
