Overview
Genital culture recovers selected bacterial pathogens from urethral, cervical, vaginal, or other genital sites using appropriate media and atmosphere.
Clinical Importance
Detect N. gonorrhoeae when culture is required for AST or legal/public-health pathways
Evaluate vaginitis/cervicitis pathogens included in culture protocols
Specimen Requirements
- Specimen: swab of the indicated site collected with validated technique
- Transport: place immediately into appropriate transport medium (e.g., Amies) unless a dry-swab molecular collection kit is specified
- Handling: transport at the temperature and time limits validated for culture or antigen/NAAT
- Site: label anatomic site clearly; interpretation depends on expected flora
- For gonorrhea culture, use validated transport (e.g., Amies charcoal) and plate promptly to chocolate/selective media (Thayer-Martin class)
Analytical Method
Specimens are inoculated to selective chocolate-based media for N. gonorrhoeae and other media per protocol, incubated in CO₂, and identified by biochemical, immunologic, MALDI-TOF, or molecular confirmation methods.

Reference Values
Microbiology results are interpreted against organism identification, specimen quality, clinical context, and current CLSI and/or EUCAST interpretive criteria (including CLSI M100 for susceptibility where adopted). Reporting conventions and critical-notification lists are institution-defined; verify against the performing laboratory SOP and current guideline editions.
Expected: no N. gonorrhoeae (and no other reportable pathogens on the ordered panel). Normal genital flora reporting follows laboratory policy.

Common Causes of Increased Results
- Pathogen isolated; confirm identification; perform AST when indicated (especially gonorrhea)
Common Causes of Decreased Results
- Negative culture . NAAT may still be positive; culture is less sensitive for many STI pathogens

Pre-analytical
- Delayed plating killing fastidious N. gonorrhoeae
- Wrong swab type for the ordered method

- Inadequate CO₂ incubation
- Misidentifying nonpathogenic Neisseria

- Reporting colonizers as pathogens without clinical/site correlation
- Wrong interpretive criteria (CLSI vs EUCAST; outdated breakpoints)
- Failure to notify critical positive cultures per institutional policy
Advantages and Limitations
Advantages
- Provides isolate for AST and confirmation
Limitations
- Lower sensitivity than NAAT for gonorrhea/chlamydia pathways
- Does not replace NAAT for Chlamydia trachomatis
Clinical Pearls
If the clinical question is gonorrhea or chlamydia screening, NAAT is usually the primary test; culture is for when you need the organism alive.
